Focused Neuro Monitoring

RNs provide high-level neurological observation and stroke-specific vital-sign tracking so changes can be identified quickly and communicated clearly to the care team.

Medication & Home Safety Support

We complete at-home medication reconciliation to reduce dosing errors and perform comprehensive fall-risk assessments tailored to neurological recovery.

Our discharge-focused care helps patients and families understand the recovery plan, recognize warning signs, and maintain a safer routine at home. Referrals are processed promptly, with an RN available to reach the patient within 24 hours after hospital discharge.
